As a LPN / RN with Martell & Co., you will:

⦁ Utilize the nursing process to assess, plan, implement and evaluate patient care.
⦁ Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
⦁ Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
⦁ Furnishes services in accordance with Martell & Co. / Home Care and Assistance’s policies and the State Nurse Practice Act.
⦁ Prepare and record clinical notes and progress notes.
⦁ Observe and report any signs or symptoms indicative of changes in the condition of the patient and family situation to the Director of Clinical Services, and document such reports and observations in the clinical record.
⦁ Performs assigned nursing procedures, including the administration of treatments and medication, in the care of the patient, under the supervision of a Registered Nurse, in accordance with the State Nurse Practice Act and the Plan of Care.
⦁ Prepares equipment and materials for treatments observing aseptic techniques as required.
⦁ Assists the patient in learning appropriate self-care techniques.
⦁ Assists with the rehabilitation of patients according to the patient care plan.
⦁ Consult the RN in making judgments or decisions regarding the care and treatments.

⦁ Must be able to pass a background check
⦁ Must be 18 years old
⦁ Must have reliable transportation with active auto insurance
⦁ Must have a valid drivers license
⦁ Graduate of an approved Practical Nursing Program and one year of nursing experience in the past five years or completion of an accredited refresher course within the past two years.
⦁ Currently licensed and in good standing as a Practical Nurse in the State of Michigan.
⦁ Knowledge and ability and the necessary skill and judgment to perform all nursing procedures as outlined in the scope of practice in the Nurse Practice Act. Understanding of the working team concept and the ability to work under supervision. Current CPR certificate.
⦁ Meets the requirements for employees providing direct patient care.
⦁ Read, write, speak and understand English as needed for the job.
⦁ Must have a valid driver’s license and use of an insured automobile for work or access to adequate transportation.
